establishes a program metric which reflects the activity and performance of the patient navigator at definite milestones throughout continuum-of-care of the patient navigator practice and provides operational benchmarking.

Develop and standardize the orientation process and performance standards
Create job descriptions and competencies for lay/clinical navigators
Identify training opportunities for existing supportive care staff
Provide a series of training for both lay/clinical navigator
Define role and scope of practice of the lay/clinical navigator
Provide a standardized presentation process for breast conference/tumor board

PROGRAM INTERNAL & EXTERNAL RESOURCES ASSESSMENT

Obtain patient perspective “flow” and barriers confronted, timeliness, and efficiency of services
Obtain physician and supportive care team perspective of navigation services (satisfaction with navigation provided, efficiency, and comprehensiveness)
Identify gaps and need for additional facility or community services to change the structure or services
